All families, new and returning need to fill out these forms. These forms will be provided to you after you have applied online.
All forms must be signed in pen (please do not type your signature in).

Forms for New Children
The forms we need for children starting their first year at Lemberg.
Lemberg uses a service called UpBup to process the State required enrollment forms. Once parents commit to enrollment, they will receive an invitation to create an account in UpBup. Electronic signatures are accepted for the UpBup forms. Please process all applicable forms to complete your child's file. If you have any difficulties accessing UpBup, please email them at UpBup service.
- For those children with allergies or chronic health conditions, parents will need to select and download the Individual Health Care Plan and Medication Consent Forms on the UpBup website, to process with their child's physician. Once these forms are processed please bring or fax to Lemberg's office.
- Current annual physical exam forms and immunization records are required for all children. Lead screening is required for children under 4 years old. Doctor's offices usually have their own forms for the physical exam results but if needed, there is a Massachusetts school form (pdf) that your doctor can fill out .
- Parents please also submit the age appropriate Ages and Stages Questionnaire* (one questionnaire only). This is a required screening filled out by parents and gives the teachers valuable information about your child. Choose the questionnaire that the closest to the age of your child at the point of time you are filling it out. For example, if your child is 26 months old, fill out the questionnaire for 27 month olds.
